Central PA Connect HIE, along with the other Pennsylvania Health Information Exchanges (HIEs), have joined the collaborative effort between state agencies, counties, local non-profits and community organizations, and healthcare and social services providers to create an integrated platform for care coordination. This platform, known as PA Navigate, will serve as a system to screen citizens for needs that impact their ability to thrive, such as childcare, transportation, nutritional support, housing insecurity and many other health related social needs.

Once needs are identified, citizens of Pennsylvania are referred to Community Based Organizations (CBOs) in their region, that can provide the identified services. CBOs are able to “close the loop” with the person making the referral so they know the citizen has received the requested services. Non-profit CBOs will have access to PA Navigate for free and will be able to use the platform to demonstrate their community impact to their funders. This platform is available to the general public so that all citizens of the commonwealth can identify available services for themselves or their families.
