Central PA Connect HIE, along with the other Pennsylvania Health Information Exchanges (HIEs), have joined the collaborative effort between state agencies, counties, local non-profits and community organizations, and health-care and social services providers to create an integrated platform for care coordination. This platform, known as PA Navigate, serves as a system to screen citizens for needs that impact their ability to thrive, such as childcare, transportation, nutritional support, housing insecurity and many other health-related social needs.

Once needs are identified, residents of Pennsylvania are referred to Community Based Organizations (CBOs) in their region, that can provide the identified services. CBOs are able to “close the loop” with the person making the referral so they know the citizen has received the requested services. Non-profit CBOs will have access to PA Navigate and for free will be able to use the platform to demonstrate their community impact to their funders. This platform is available to the general public so that all residents of the commonwealth can identify available services for themselves or their families.

For PA residents

• Enter the Zip Code of relevance to find services of interest close to the individual in need. Some categories of Available Services are listed above.

For health-care organizations interested in using PA navigate to screen individuals and make electronic referrals

• In most cases, health-care organizations that want to fully leverage PA Navigate will be able to integrate the referral platform with their existing electronic health-care record, care management platform, or other system of record.

For community-based organizations

• Use of PA Navigate is offered at no cost for non-profit CBOs.
• A CBO can easily join PA Navigate and claim its listing in the service directory and opt in to receiving electronic referrals from participating health-care organizations.
• CBOs can use the PA Navigate platform to demonstrate their community impact to their funders.
• To sign up, visit the PA Navigate Website and click Sign Up in the top, right-hand corner.
• If your program does not exist, click the Suggest a Program link at the bottom of the screen to get your program added to our database.
